Fjarlækningar handles common issues that can be resolved without an in-person examination by a doctor. This makes the path to care simpler and safer, while reducing waiting times for medical services.

The process from start to finish

The entire process is developed in collaboration with specialist physicians and doctors with decades of experience in primary care. The questionnaires reflect the questions a doctor would ask a patient, but the key difference is that you can answer them from anywhere. A doctor then reviews your answers and determines the next steps based on their medical assessment.

    You choose a concern from the list of issues

    Log in with electronic ID and follow the relevant pathway based on your symptoms.

    You answer a questionnaire

    The questionnaires are tailored in collaboration with Icelandic specialist doctors for each issue — the process is designed like a consultation with a doctor. When a home test improves diagnostic assessment, you will be guided to take one, such as a urine test available from primary care or your nearest pharmacy, and enter the result directly into the portal.

    Red flags assessed in combination with your answers

    Red flags in the questionnaires immediately direct serious symptoms to the right service. Fjarlækningar does not handle concerns that belong elsewhere — and you do not pay if you are redirected.

    A doctor reviews and recommends treatment

    A doctor reviews your answers and recommends appropriate treatment based on their medical assessment. No treatment without a doctor’s assessment.

    Outcome, advice, and prescription

    You receive a written outcome and advice, and if needed, a prescription is sent electronically to the medication portal — ready at your nearest pharmacy. The outcome also includes educational material related to your concern: advice, preventive guidance, and over-the-counter products that may help.

Home tests that may be part of your case

For some cases, a simple self-test improves the assessment. The questions from Fjarlækningar doctors will indicate whether a home test is needed, but this does not apply to all cases. You collect the self-test yourself, take it where convenient, and enter the result.

Why self-tests?

For common, clearly defined cases, a simple test that you take yourself can provide the doctor with the information needed to complete the assessment — without having to book an appointment, travel, or wait. This makes the process simpler and faster for you, while allowing primary care to focus on more complex cases.

When a self-test improves diagnosis, you will be guided to take one, and the result increases diagnostic accuracy. However, the test does not replace a doctor’s assessment: a doctor reviews the result together with your questionnaire answers. If the result is unclear, or your symptoms do not match it, your case is referred to conventional care.

How it works in the patient portal

    Questions determine whether a self-test is needed

    You choose a case type and answer standard questions, which takes a few minutes. If a self-test is relevant for your case, you will receive information about which test is needed, but this does not apply to all cases.

    You collect the self-test — complete it where convenient

    You can collect the self-test from your health centre or a pharmacy — see above for the most convenient option. Instructions for how to take it are included in the package and in the patient portal.

  3. 3

    You enter the result in the patient portal

    You enter the result directly into the patient portal. All information about your case and the test is sent to a doctor for review, and the doctor completes the case.

When is telemedicine not suitable?

Fjarlækningar handles simple, clearly defined cases. Some cases require an examination, testing, or urgent care and are therefore referred through another care pathway.

Serious symptoms

Cases with serious symptoms are referred through another care pathway.

Serious symptoms may include:

  • Chest pain or pressure in the chest
  • Breathing difficulties or sudden shortness of breath
  • Sudden weakness, numbness, speech disturbance or paralysis (stroke symptoms)
  • Reduced consciousness or fainting
  • Heavy bleeding or serious injuries
  • Sudden, unbearable headache or abdominal pain
  • High fever with neck stiffness or skin bleeding
  • Severe allergic reactions (swelling of the face/throat, breathing difficulties)
  • Thoughts of self-harm or suicide

If you have serious symptoms, you can:

  • Call 1700 for advice from a nurse
  • Seek care at Læknavaktin (in the Capital Region)
  • If it cannot wait: go to the Emergency Department or call 112

Need for examination or testing

If a patient needs an examination or further testing, the doctor refers them back to conventional care.

Initial diagnosis

The initial diagnosis of certain conditions is referred through another care pathway. Cold sores and shingles of the skin are examples: an initial diagnosis requires a doctor’s assessment with an examination, but recurring symptoms that you recognize can be handled here.

Serious conditions in your medical history

If your medical history shows serious underlying conditions, it may be too risky to manage the case through telemedicine — in that case, you are referred to conventional care.

The service

How does the medical service work?

Medical services from Fjarlækningar doctors are available through the Fjarlækningar website, wherever it suits you. There, you can access treatment options for defined, common conditions. With Fjarlækningar, you get a more convenient way to see a doctor, with a medical assessment — and a prescription if appropriate — within 2 hours every day between 10:00–22:00.

You go through a process where you answer a doctor’s standard questions about yourself and your health. Once you have completed the questions, you pay the same fee as a visit to primary care, and your case is sent to a doctor. A Fjarlækningar doctor provides a tailored treatment plan, and your outcome appears in the patient portal, where you can access treatment options and messages from the doctor. If the diagnosis indicates that you need a prescription medicine, a prescription will also be sent to the medication portal, where it can be redeemed at your nearest pharmacy.

Fjarlækningar provides medical services for the following conditions:

  • Medical certificatesSick notes for employers and schools. This service is only available to those who need an absence certificate related to a condition that has been handled through the telemedicine service.
How much do Fjarlækningar medical services cost?

Fjarlækningar medical services are comparable to a primary care visit, and you only pay the standard visit fee of ISK 1,000 per case, just as in primary care. Fees for sick notes follow the primary care fee schedule.

How can I use the medical service?

Fjarlækningar medical services are available through the Fjarlækningar website by clicking “patient portal” and through the HSU website: https://island.is/s/hsu
